The Role
We are currently seeking an experienced Social Worker (Grade 2) to work in our Acute team at Malvern and Brighton on either a part time or full time basis to provide psychosocial care and support discharge planning for our inpatient services. Flexibility to work with our palliative and rehabilitation services across Malvern and Brighton sites as required.
This role will also have opportunity to be involved in service and professional development activities within the team. This will include supervision of students, Allied Health Assistant and Grade 1 Social Workers.
You will be supported by a strong social work team, which is committed to providing high-quality, patient-centred, and evidence-based services within a supportive environment.
Key responsibilities include:

You are able to establish positive and collaborative working relationships with hospital staff, patients and families, utilising your excellent communication skills.

Maintain conflict resolution and negotiation skills, whilst achieving positive outcomes for patients and families.

Manage your complex caseload effectively and facilitate timely discharges for patients from hospital.

Consult and collaborate with others and work as an effective member of team to deliver organisational outcomes.

Demonstrate your ability to undertake professional supervision and/or mentor junior staff, and supervise students.

Have flexibility, innovation, creativity and optimism.

Demonstrate ability to develop evidence-based and innovative practices and projects.
About you
The successful candidate will be a dynamic qualified Grade 2 Social Worker.

Bringing sound knowledge of social work theories and practices in accordance with AASW Guidelines and Code of Ethics.

Clinical practice experience in the healthcare setting (minimum of 3 years).

A 'can do' attitude and ability to manage competing demands.

Eligible for membership of the AASW.

Desirable: Experience in hospital based care and knowledge of private hospital sector.
